Secure Contact Form - Stop Users From Spamming Your Forms

I've been receiving a ton of spam attempts through my unsecure contact forms, so I've started implementing an image captcha on my forms. I'm offering this same option for anyone looking to limit the amount of spam they have been receiving through their forms and from users who try to manipulate your form to send spam to others.

What this form does:

- Sends Email Verification to the email address set in config file
- Uses php include file for easy form loading into any page
- You can add as many form fields as you want (editing html file)
- It logs the submitter's ip address by default
- You can define the subject of the email
- It does not submit unless the image captcha is correct
- It validates required form fields
- Uses stylesheet for form fields but you can use your own
- Sends user to successfully completed page upon submission
- Sends user to failure page if error occurs

You can use the form as you wish, it is best used as a support or contact form to limit bot submissions. I've been receiving many, but I'm now implementing this on all of my sites.

This form can load in ANY of your pre-developed pages, allowing it to blend nicely into your design. The only requirement is you must ensure the page ends with .php extension rather than .html for it to function correctly.
